Including Monash Health in your Will

An example of wording you may choose to use for a general bequest:
“I bequeath to Monash Health of 246 Clayton Road Clayton, Victoria 3168 (ABN 82 142 080 338) or any successor, the sum of ________________________________________ dollars OR _________________________________% of my residual estate to be applied by Monash Health in such a manner as it may determine.
I direct that the receipt in writing of an authorised officer of Monash Health shall be deemed proof of payment.”
Specific purpose wording
“I bequeath to Monash Health of 246 Clayton Road Clayton, Victoria 3168 (ABN 82 142 080 338) or any successor, the sum of ________________________________________ dollars OR _________________________________% of my residual estate to be applied for the following purpose ______________________ (please insert specific purpose).
If at any time Monash Health determines that it is impossible or impractical to fulfil the purpose of my bequest, the Monash Health may apply the bequest for a related purpose that most closely aligns with the spirit and intent of my bequest.
I direct that the receipt in writing of an authorised officer of Monash Health shall be deemed proof of payment.”
